The 2021–22 First League of the Republika Srpska was the twenty-seventh season of the First League of the Republika Srpska, the second tier football league of Bosnia and Herzegovina, since its original establishment and the twentieth as a second-tier league. The season began on 13 August 2021 and ended on 28 May 2022.

Notes:
  1. ^ Krupa were prevented from promotion to the Premier League due to failing to obtain a license for it.[1]
  2. ^ Krupa were deducted 1 point due to their youth team not playing in the national division.[2]
  3. ^ Sloga Doboj were given a license to compete in the Premier League and were selected as Krupa's replacement.[3]
  4. ^ Borac Kozarska Dubica were deducted 3 points.
